By consenting to this program, you agree to participate in the following initiatives:
- Create an inclusive environment for all participants within your school, program and community
- Provide the opportunity for at least one Special Olympics sporting opportunity within your school
- Add Special Olympics Minnesota logo to printed materials, t-shirts, etc.
- Commit one teacher or staff member to serve as the liaison or point of contact for the Special Olympics Minnesota state office
- Agree to allow Special Olympics Minnesota to use any and all photos of said athletes, Unified partners and coaches unless otherwise noted (photos will be used for related publications and communications)
- Complete mid-year and end-of-the-year reports provided by Special Olympics Minnesota
It is understood that Special Olympics Minnesota will provide:
- A partnership to help schools and programs provide quality sporting opportunities for young adults with intellectual disabilities
- Consultation, education, support and resource assistance for all sporting programs and activities that promote inclusion, acceptance, respect and dignity for all participants
- Resources to support school based Unified School activities
- Leadership training and networking for educational leaders and students
- Guidance on Special Olympics Minnesota and Unified Schools policies and procedures, training manuals, schedules, paperwork and other initiative information
In addition, the school or program can raise and expend funds under the name of Special Olympics. The liaison will comply with Centralized Accounting policies as determined necessary by the state office. All monies raised and expended in the name of Special Olympics will be documented according to accounting policies and procedures set by the state office.
I hereby certify that I have read the consent & waiver text and that my school or program agrees to become partner with Special Olympics Minnesota.
